Scouting Annual Medical Form (Part ABC) 

Camp Winton 2026 Forms

2026 Winton Merit Badges and Program Offerings 

For Camp Winton, each scout will need all of the below forms completed and turned in to the troop:

1. Copy of Annual Health and Medical Form ABC (Part C MUST be signed by a physician)

***Include with medical Form a copy of the scout's insurance card and immunization record

2. Trip/ Activity permission slip

3. Climbing permission form

4. Range and Target Activities permission form

5. Early release permission form (if applicable, only needed if Scout is planning to leave camp early)

6. Scouts should bring their Totin' Chips and Firem'n Chit. White cards are not needed to use the range at camp and should be left at home.
